Case Manager – Upland, CA

Our client is a different kind of Medicare Advantage health plan designed to meet special needs of the senior community. The ideal candidate will be an RN or LVN with at least 1 solid year of case management experience and a strong clinical background. 

Required Qualifications

• RN or LVN
• At least 1 year of case management experience at a health plan, medical group or hospital.
• Strong clinical background with knowledge of multiple disease processes 

Preferred Qualifications

• CCM or other case management certification is preferred.
