REQUIRED 2 years of experience. Job Summary Assesses the health status of assigned patients on a regular basis. Manages medical regimen of patients within limits of established protocols. Evaluates care and coordinates interventions for assigned patients in conjunction with all members of the health care team. Participates in interdisciplinary team meetings and care planning conferences and communicates with team/patient/family concerning medical assessment, plans for treatment, and patient progress. Coordinates care or provides appropriate services/levels of care across continuum of care (acute and long term). Coordinates data collection efforts and implements outcomes monitoring to evaluate program effectiveness. Provides home health visits for residents when indicated. Performs as practitioner, educator, consultant and researcher. Documents pertinent clinical information, statistical information and appropriate coding and reimbursement information. Develops strong interpersonal relationships with assigned patients, families, health care providers and other team members. Job Qualifications Graduation from an accredited Physician Assistant Program. Approval and licensure by the North Carolina Board of Medical Examiners. Certification by the National Commission on Certification of Physician Assistants required.
