The Case Manager-Mental Health Therapist directly coordinates & monitors a complete rehabilitation program.   Functions as an effective team leader.  Writes client progress reports, & is responsible for all communication regarding client and program information. Responsible for evaluating clients and developing plans of care with measurable short-term objectives and long-term goals.
 
Core Qualifications:
MI Licensure to practice psychology, social work or counseling
Master's degree or higher in social work, psychology or counseling
Willingness to be on-call
Leadership skills
Strong written & verbal communication skills
Strong interpersonal skills
Ability & willingness to learn new skills
Ability & willingness to set priorities
Ability to work as part on an interdisciplinary team
Ability to lift, maneuver & carry up to 30lbs.
Valid driver's license
Acceptable driving record
Acceptable under the State of MI Good Moral Character Statute
Knowledgeable of HIPAA Privacy Rules
 
Addl Qualifications:
Experience working the TBI population preferred
