Hospice Community Liaison (closed)
A leading patient and family focused Central Ohio Hospice organization is seeking a Community Hospice Liaison. The Community Liaison is responsible for generating patient census by identifying, developing and maintaining relationships with physicians, long term care facilities, social service organizations and all other local referral sources to meet territory and agency revenue and census goals. The Community Liaison will also be responsible for developing and implementing programs that raise community awareness for hospice and its services by representing the organization at community events.
The ideal candidate must be self motivated, have medical or healthcare experience (Home Care, Hospice or Long Term Care preferred), excellent communication skills and a desire to be a member of a Team of Healthcare professionals committed to providing the highest quality of care.
