Job Description

Assists in the planning, creating, training in and supervising of the activities/goals/outcomes/services provided in the adult day services group room. Observes and analyzes the individualized needs of the people in Adult Day Services.  Coordinate the creation of methodologies, goals/outcomes, services to enhance skills, through the Team process. Helps develop individual plans, and thereafter develop annual (or as frequently as necessary) evaluations of the individuals’ progress and needs.   Prepare, schedule, conduct and document all consumer meetings.   Observe, assesses, and documents monthly progress notes per policy.   Responsible for supervising group leaders in the formulation and implementation of individual service plans, and the planning and conducting of  meetings involving the individuals on their list of responsibilities.   Maintain contact with families, correspondents, state agencies or other entities entitled to be involved in the life of the consumer.    Instruct and supervises staff in carrying out the programs as written.   Assist the Group Room staff in helping consumers achieve the highest level of independence in all areas of daily living, by implementing daily goals and activities, including but not limited to:  self care (including toileting), communication, socialization, eating, cognitive skills and mobility.   Review monthly data sheets and daily attendance records to ensure accuracy and compliance with regulatory and billing standards.   Conduct initial and continued in-house orientation training for direct support level staff   Ensure that all program charts remain in compliance; respond to all program audits.   Act to insure the safety and well-being of all consumers in the Program.   Arrange staff assignments based on planned or unplanned absences of staff.  Coordinate all daily activities with the approval of Administration.   Demonstrate appropriate interactions between co-workers and consumers in accordance with OPWDD regulations.   May be asked to accompany individuals on community inclusion outings which may involve public as well as private transportation.  Oversee the arrangements of such outings.   Mandatory attendance at all required trainings and in-services. Must demonstrate understanding and ensure compliance to OPwDD regulations.    Attendance/Punctuality:  is consistently at work and on time.  Ensures work responsibilities are covered when absent.  Arrive at meetings and appointments on time.      

Supervisory Responsibilities:   Supervises all Group Room staff including temporary staff.           

MUST HAVE A BACHELORS AND EXPERIENCE WORKING WITH DEVELOPMENTALLY DISABLED INDIVIDUALS.
