Large Tulsa clinic is hiring a Case Worker/Social Outreach Coordinator. Position will assess patient psychosocial needs and develop a plan; screen patients for appropriate programs and benefits that they may qualify for, and assist potentially eligible patients as needed in applying for these programs; Coordinate and attend clinic outreach activities. Two year degree in Social Sciences from college or technical school and 2+ years experience required. Bachelor's degree in Social Work and bilingual (English/Spanish) preferred. Candidates must have excellent oral and written communication skill as this position will also act as the clinic ambassador by fostering relationships.
