Job Description
| Department: NURSING-DIRECT PATIENT CARE - Full-time 8 - Days/weekends - Salary Range: Grade 40 $29.19 Pay will commensurate with experience Responsible for provision of primary care, health education and promotion, disease prevention, care coordination and continuity of care for patients accessing Functional Medicine and Central Georgia Rehab Hospital. Care is delivered based upon established protocols. Facilitates continuity of care delivery by serving as a practitioner, manager, consultant/leader, and change agent within Functional Medicine and CGRH. Position requires excellent interpersonal skills, as collaboration with multi-disciplinary colleagues and interactions with a diverse population will be necessary. Accountable for assessment, planning, implementation and evaluation of patient care as it relates to nursing standards and nursing plans for the provision of care. Education: BSN required. Master’s degree from an approved program for nurse practitioners required. Experience: Minimum of three years nursing experience. Minimum of one year as a nurse practitioner preferred. Certification/License/Registration: Must be eligible for or certified as a Nurse Practitioner in area of expertise. |
