Job Description

Leading New York City Managed Care Company seeks a Registered Nurse (RN) – Telephonic Case Manager to identify, assess and manage medically cases per established criteria. 
 
Responsibilities include developing case management plan of care consistent with the sound medical and financial management, conducting assessment of health needs, individualized case management plans, implementation, monitoring and evaluation of case outcomes using outcome driven case management model, and arranging for health care services within the scope of available benefits.

Additional responsibilities include coordinating case management activities with the primary care provider, UM staff, hospital discharge planner, social workers, public agencies, and other providers as required, working with participating ancillary service providers and public agencies as appropriate to ensure quality cost effective care for the member, and . Build relationships with member’s provider.

Qualified individuals will be a Licensed Registered Nurse with at least 2 years of case management experience, preferably in an managed care setting.
